Job Description

Are you a detail-oriented professional with a passion for helping others navigate the healthcare system? We are seeking a Care Coordination Support specialist.  In this role, you will serve as the administrative backbone for our clinical staff, acting as the primary point of contact for members, providers, and internal teams. This position is ideal for someone who thrives in a fast-paced environment, excels at multi-tasking, and is dedicated to ensuring members receive high-quality service and timely access to care. As a remote team member, you will play a critical part in coordinating life-changing waiver services and addressing the social determinants of health that impact our community. ***Candidates can be local to the following areas: Waco, TX, Killeen, TX, Bryan, TX, College Station, TX, Temple, TX or Corpus Christi, TX***

salary: $17 - $19 per hour

work hours: 8 AM - 5 PM
education: High School

Responsibilities

  • Member Outreach & Support: Perform telephonic outreach to assist members with care plan next steps, educate them on available benefits, and resolve inquiries or concerns.
  • Service Coordination: Schedule medical appointments, arrange member transportation, and conduct provider searches to ensure high-quality care delivery.
  • Administrative Facilitation: Manage member onboarding by sending welcome letters and program materials; document all interactions in non-clinical records to maintain state compliance.
  • Clinical Team Collaboration: Task new requests to Social Workers for Medical Necessity (MN) determinations and follow up on pending services within a 28-day window.
  • Issue Resolution: Handle "Unable to Locate" (UTL) processes, manage provider termination notifications, and address Social Determinants of Health (SDOH) by making local resource referrals.
